- MMT Ambulance will be collecting opt-in verbally. When a caller is registered for the first time, they are asked to provide the phone number and will be asked if the caller would like to opt in to customer care notifications. They will be verbally informed that "Message and data rates may apply", "Message frequency may vary on the type of their concern", and they can "text HELP for support or more information and STOP to unsubscribe at any time." They will also be informed that their phone number will not be shared with third parties for marketing or promotional purposes. After completing this process, you acknowledge and represent to Midwest Medical Transport Company, LLC (“MMT” or “we” or “us”) that you are the current wireless service plan subscriber and/or an authorized user of the mobile phone number(s) that you enroll in the Program, or you have been granted permission by the wireless service plan subscriber and/or authorized user of the mobile phone number(s) to enroll the mobile phone number(s) in the Program.
- You grant MMT express permission to send automated text messages to the enrolled mobile phone number(s) through your wireless phone carrier unless and until such permission is revoked in accordance with these SMS Terms and Conditions.
- By granting such permission to MMT, you are hereby requesting to receive text messages in spite of the fact that your mobile phone number(s) may otherwise be on the federal, or a state's, Do Not Call List and you agree that, to the maximum extent permitted by law, your request overrides any prior Do Not Call request for or related to the Program. You are not required to agree to receipt of text messages in order to purchase any product or service from MMT.
- We will not be liable for any delays in the receipt of any SMS messages, as delivery is subject to effective transmission from your mobile service operator. SMS MESSAGE SERVICES ARE PROVIDED ON AN “AS IS” BASIS, AND WE MAKE NO WARRANTY, EXPRESS OR IMPLIED, AND ALL WARRANTIES, INCLUDING IMPLIED WARRANTIES OF MERCHANTABILITY AND FITNESS FOR PARTICULAR PURPOSE, ARE HEREBY EXPRESSLY DISCLAIMED.
- Data obtained from you in connection with this Program may include your cell phone number, your carrier’s name, and the date, time and content of your messages, as well as other information that you provide. We may use this information to contact you and to provide the services you request from us. No mobile information will be shared with or sold to third parties for marketing/promotional purposes that has been collected through dedicated verbal opt-ins without consent.
- By enrolling in the Program, you consent to receive text messages, including alerts and updates. Message frequency varies.
- By enrolling in the Program, you acknowledge and agree that we will have the right to change and/or terminate the service at any time, with or without cause and/or advance notice.
- To cancel your SMS subscriptions, email [email protected] with the message “remove me from SMS messaging”. You may receive a subsequent email confirming your opt-out request.

- Program Availability: Currently, the Program is only available to residents of the United States. You understand and acknowledge that you may not sign up for, access, or attempt to access or use the Program from countries outside of the U.S. You agree to abide by U.S. and other applicable export control laws and not to transfer, by electronic transmission or otherwise, any content or software subject to restrictions under such laws to a national destination or person prohibited under such laws.
